Le copiste Siméon de Mîdo Qastra se nomme au début et à la fin du manuscrit, la signature de l'artiste est placée entre l'index des leçons et les évangiles au f. 10v (voir Fr. Nau dans Journal Asiatique, 11e série, V, 1915, p. 501.
Une note en fin de volume dit que ce manuscrit a été acheté par deux moines, Lazare et Gabriel, fils de Joseph, en 1509 de l'ère des Séleucides (1198 de n. è.)
